Who is responsible

Dr Glenda Lattimore is responsible for enquiries to Living Well Telehealth and is your privacy contact. Email [email protected] and mark your message “Privacy”. This notice covers this website and appointment enquiries. Your psychologist provides information about clinical records and treatment consent before care begins.

What we collect and why

We collect the details you enter: your name, email, optional phone number, contact preference, appointment type, session format, preferred times and expected payment arrangement. Because an enquiry identifies you as seeking psychology services, we treat it as health information.

We use it to reply, consider whether the service is suitable, arrange appointments and manage related administration. You can use a first or preferred name initially. Without a way to contact you, we cannot reply. You can email us instead of using the form, although ordinary email also uses third-party services.

Please keep clinical history, Medicare numbers and referral documents out of website enquiries and ordinary email. We will explain how to provide any documents through an agreed clinical channel. The form has no document upload or payment fields.

Who can see your enquiry

Enquiries reach the practice mailbox. Dr Lattimore and authorised administrative or technical support may access them only as needed to arrange care, maintain the service or resolve a problem. We do not sell enquiries or use them for advertising or mailing lists. The website does not automatically send enquiries to an AI tool or make clinical decisions.

Our service providers process information to deliver and protect the service. Information may also be disclosed where required or authorised by law. We ask for your agreement before you submit an enquiry; this is separate from consent to psychological treatment.

Our providers and overseas processing

Cloudflare provides website hosting and Turnstile spam protection. Resend sends enquiry and acknowledgement emails, and Google Workspace stores the practice mailbox. These services may process personal information outside Australia. Resend routes our emails through Japan and stores account data, including email metadata, logs and API records, in the United States. Google and Cloudflare use international infrastructure and support teams; relevant locations include the United States, European countries and other countries listed in their current provider disclosures. We do not promise Australian-only storage.

Provider details: Cloudflare privacy, Resend processing locations, and Google Workspace processing locations. We use these services to operate the website and email, while remaining responsible for how we handle your information.

Security, cookies and technical information

The website uses HTTPS and the form uses server-side validation and spam protection. It does not keep an enquiry database or deliberately log the contents of your enquiry. Enquiries are delivered by email and retained in the practice mailbox and by the email provider.

Cloudflare may process your IP address, browser and device information, request timing and security signals to deliver the site and prevent misuse. Security cookies may be used for these functions. We have not added advertising trackers or third-party audience analytics. No website or email system can guarantee absolute security; please send only the information requested.

How long information is kept

We keep enquiries only while needed to respond, administer care or meet legal and professional obligations. We review closed enquiries that did not lead to a health service after 12 months and remove them when no longer needed. An enquiry involving assessment, advice or treatment may form part of a clinical record and is retained accordingly.

For adult clinical records, NSW requirements generally mean keeping health information for at least seven years after the last health service. Information collected while a person was under 18 must be retained at least until they turn 25. A complaint, claim or other legal obligation may require longer retention. Records are securely disposed of when eligible, with any required disposal record retained.

Resend currently lists 30-day email data retention on our plan. This is separate from the practice mailbox, security records and providers’ other legal or operational records; it does not mean your enquiry is deleted everywhere after 30 days.

Access, corrections and your choices

Email [email protected] to ask what we hold, request a copy or correction, or ask us to stop contacting you. We may need to verify your identity and will explain a safe way to do this. We aim to respond within 30 days. If access or correction is restricted by law, we explain the reason and available complaint options. Withdrawing consent does not undo information already used or require destruction of records we must legally retain.

Privacy concerns or complaints

Contact Dr Lattimore at [email protected] with your concern and the outcome you seek. We will acknowledge it, investigate and aim to give a written response within 30 days, or explain any delay. Please request a secure way to provide sensitive supporting documents.

If you remain dissatisfied, you can contact the Office of the Australian Information Commissioner or the Information and Privacy Commission NSW. You can also ask us for a copy of this notice by email. We update this page when our enquiry process changes.
